    Looking for a time independent Hamiltonian of a dynamical system

    In this paper we introduce a method for finding a time independent Hamiltonian of a given dynamical system by canonoid transformation. We also find a condition that the system should satisfy to have an equivalent time independent formulation. We study the example of damped oscillator and give the new time independent Hamiltonian for it, which has the property of tending to the standard Hamiltonian of the harmonic oscillator as damping goes to zero.Comment: Some references added, LATEX fixing

    The role of the occupational therapist in disaster areas: systematic review

    Background. Disasters are increasingly more frequent events on our planet. During disaster the role of the occupational therapist will require a more specific operative framework within nongovernmental organizations and community health services. Design. Systematic review. Objective. The aim of this study is to evaluate the evidence that highlight occupational therapist’s role in disaster area through a systematic review. Materials and Methods. Research on MEDLINE was performed. All articles from 2005 to 2015 concerning rehabilitation and occupational therapy in disaster areas were included. Results. Ten studies were selected to be included in this review. Four interesting points emerged: the importance of having rehabilitation intervention in postdisaster situations, the necessity to include a rehabilitation team in the early phase of disaster response, the need to provide a method to address the difficult evacuation, and finding the safest method of transport of people with preexisting disabilities and new injuries. Conclusions. The amount of evidence with respect to specific intervention of the occupational therapist’s role in a disaster situation is limited. However some evidence suggests that it could be a good means for reducing the number of medical complications and deaths of persons with preexisting disabilities. The evidences found highlight the necessity to create a multidisciplinary team addressing needs in disasters situation, in which the occupational therapist could certainly contribute

    The Barthel index: italian translation, adaptation and validation

    The Barthel Index (BI) is widely used to measure disability also in Italy, although a validated and culturally adapted Italian version of BI has not been produced yet. This article describes the translation and cultural adaptation into Italian of the original 10-item version of BI, and reports the procedures for testing its validity and reliability. The cultural adaptation and validation process was based on data from a cohort of disabled patients from two different Rehabilitation Centers in Rome, Italy. Forward and backward translation method was adopted by qualified linguist and independent native English official translators. The scale obtained was reviewed by 20 experts in psychometric sciences. The Italian adapted version of the BI was then produced and validated. A total number of 180 patients were submitted to the adapted scale for testing its acceptability and internal consistency. The total time of compilation was 5 ± 2,6 minutes (range 3-10). Validation of the scale was performed by 7 trained professional therapists that submitted both the translated and the adapted versions to a group of 62 clinically stable patients (T-test=-2.051 p=0.05). The internal consistency by Cronbach’s alpha resulted equal to 0.96. Test – retest intra – rater reliability was evaluated on 35 cases; at test-retest was ICC=0.983 (95%IC: 0.967-0.992). This is the first study that reports translation, adaptation and validation of the BI in Italian language. It provides a new tool for professionals to measure functional disability when appraising Italian speaking disable patients in health and social care settings along the continuum of care

    Low-density lipoprotein-lowering medication and platelet function

    Elevated low-density lipoprotein (LDL) cholesterol (LDL-C) levels represent one of the most important risk factors for atherosclerosis and therefore cardiovascular morbidity and mortality. LDL-C operates at different levels and through various classic and non-classic mechanisms. In particular, increased or modified LDL enhances platelet function and increases sensitivity of platelets to several naturally occurring agonists. Agents that lower LDL-C in hypercholesterolemic patients have been shown to interfere with platelet function. Several studies, in fact, suggested that statins exert anti-thrombotic effects largely as a result of an anti-platelet activity. Among the other LDL-C-lowering agents those acting by interfering with cholesterol reabsorption from the gut (cholestyramine, colestipol) do not appear to interfere with platelet function, whereas peroxisome proliferator-activated receptor agonists (such as fibrates) can inhibit platelet function. The full potential of these drugs in vascular protection is only just being realized. Further studies are still needed to elucidate the full therapeutic benefits of these agents in plaque stabilization and thrombosis. Copyright (c) 2006 S. Karger AG, Basel

    Isotopic Grand Unification with the Inclusion of Gravity (revised version)

    We introduce a dual lifting of unified gauge theories, the first characterized by the isotopies, which are axiom- preserving maps into broader structures with positive-definite generalized units used for the representation of matter under the isotopies of the Poincare' symmetry, and the second characterized by the isodualities, which are anti-isomorphic maps with negative-definite generalized units used for the representation of antimatter under the isodualities of the Poincare' symmetry. We then submit, apparently for the first time, a novel grand unification with the inclusion of gravity for matter embedded in the generalized positive-definite units of unified gauge theories while gravity for antimatter is embedded in the isodual isounit. We then show that the proposed grand unification provides realistic possibilities for a resolution of the axiomatic incompatibilities between gravitation and electroweak interactions due to curvature, antimatter and the fundamental space-time symmetries.Comment: 20 pages, Latex, revised in various details and with added reference

    Gait analysis advancements: rehabilitation value and new perspectives from forensic application

    The clinical and rehabilitation value of gait analysis is remarkable and indisputable and poised to grow as technological advancements unfold. This article aims to shed light on the advances in how gait is assessed, enabling those who have suffered an injury impairing their motor skills to be diagnosed more accurately and efficiently as well as to compare the hallmarks of rehabilitative and forensic gait analysis. The authors have conducted an analysis of relevant papers (published between 1967 and 2020) from a medicolegal perspective, cited in PubMed, MEDLINE, Cochrane Library, EMBASE, and available recommendations for the legal application of such techniques. Moreover, considering the use of gait analysis as a forensic tool, this study broadens the scope of research by including search engines, legal databases, and court filings (DeJure, Lexis Nexis, Justia) between 2000 and 2022. The instrumental assessment of movement (Gait Analysis) has come to constitute an essential analytical tool for the biomedical sector to objectively and accurately assess human movement and posture. The article is also aimed at elaborating differences and similarities between clinical and forensic gait analysis. When it comes to the forensic applicability of gait analysis and its evidentiary value, however, there is a pressing need for a review of its scientific basis. Therefore, it is necessary to conduct a thorough evaluation of its use in legal practice, as stressed in scientific literature and surveys. It is of utmost importance to highlight the procedural and assessment standards currently applied to forensic gait analysis, to evaluate its strengths and weaknesses, and to achieve standardized guidelines based on broad scientific consensus

    Mechanical pull-out test of a new hybrid fixture-abutment connection: an in vitro study

    Implant abutment connection was described among the main causes of peri-implant bone resorption. The aim of this in vitro study was to test the coupling capacity, the surface modification of a new hybrid connection and the influence of repeated connection activations caused during the main clinical and laboratory phases. A total of 40 implant-abutment screw retained systems with 10 degrees-conical and internal hexagon connection were tested. The connection was screwed, fixed to the universal test machine, removed the screw and a pull-out test was performed. Test was repeated five times in succession. Also Scanning Electron Microscopy (SEM) was used to detect microscopically surface modification. Analysis of variance and Tukey tests were used for the statistical analysis. Pull-out test reveals a mean value of 131.35 +/- 16.52 Newton Centimeter (N center dot cm). For each single activation, results from first to fifth were: 113.9 +/- 13.02, 126.1 +/- 12.81, 138.11 +/- 15.15, 138.8 +/- 11.90 and 140 +/- 12.99 N center dot cm. A statistically significant difference between the measurements and an increase in the removal force was shown. The collected data supports the use of this new type of connection, resulting in a very strong interface between implant and abutment. Also, repeated activation of connection can promote a better coupling of the implant-abutment interface

    Laser and thermal therapy in athletes’ tennis elbow: an observational study

    BACKGROUND: Lateral epicondylitis (LE), is a widespread painful situation in the upper extremity that usually occurs due to overuse of the wrist extensor muscles. The aim of our study was to investigate the analgesic and functional effects of laser therapy in association with thermal therapy in sports patients suffering from lateral epicondylitis. METHODS: We retrospectively analyzed amateur tennis patients who played sports at least 3 times a week, with medical and ultrasound diagnosis of subacute lateral epicondylitis. All patients underwent 6 sessions of Laser therapy with qmd® Helios laser. A group of patients instead (23, Thermal-Laser Group, TLG), underwent a session of Thermal therapy (qmd® Helios cryo-thermal) before the Laser therapy. The Laser Group (21, LG) on the other hand only performed laser therapy. Patients included in the study were evaluated at baseline (T0), at the end of treatment (T1) and after 15 days from the end of treatment (T2 through Visual Analogue Scale-VAS, muscle strength (Dynamometer) and function (Disabilities of the Arm, Shoulder and Hand-DASH questionnaire). RESULTS: Fifty-two patients (23 M, 21 F) were enrolled (21 LG; 23 TLG). All evaluations performed show significant improvement over time in both groups. The TLG shows at T1 a greater increase in force compared to the LG. CONCLUSIONS: Laser therapy has certainly been an effective intervention in the management of epicondyle pain for some time. More studies with a higher number of patients and longer follow-up are needed in order to better characterize the benefits of this combination therapy
